Description: This is a case of dead bag syndrome (where the lens capsule is completely devoid of cellular material or scar tissue and the IOL can be free-floating in the bag) where the lens is slightly dislocated/loose. I had done an iol exchange about 3 years ago for a single piece free floating iol in the bag and placed a 3 piece iol in the sulcus with optic capture and it seemed stable. But the patient recently came back and there is a fair amount of loose zonules nasally and the bag seems to be folded around the lens. I removed the dead bag from the eye and fixated the lens with the double needle Yamane scleral fixation technique and then did an anterior vitrectomy.
