Brief Description: This is a case presentation of a middle aged male who presented with diplopia and ptosis. Differential diagnosis of pupil-involved and pupil-sparing third nerve palsies is given. When to order neuroimaging for third nerve palsy is discussed. A discussion of the diagnosis and features of myasthenia gravis are undertaken.
