Brief Description: This is a case presentation of a middle-aged male with strabismus and diplopia. The diagnosis of congenital 4th nerve palsy and possible superior oblique tendon laxity was discussed. A discussion of the surgical techniques for repairing congenital superior oblique palsy is discussed. Years later, the patient re-presented with diplopia and cyclotorsion – an extensive discussion of the surgical techniques for treating complex strabismus is discussed.
